HOW IT WORKS:
- A patient’s doctor orders medical services – such as nursing or physical therapy – so that the appropriate care can be delivered in the home.
- Within 24 hours, a PHCN representative or network provider will call the patient to verify critical information such as address and contract information – answer questions and schedule the first in-home visit.
- At the agreed-upon time, a nurse or other medical health care professional from a PHCN credentialed agency will visit a patient at home and begin care.
Patients do not typically incur any additional cost for in-home care through PHCN. This is dependent on the health plan Evidence of Coverage the patient is covered under.
